Physical Forms - All Physical Forms and Athletic Forms can be found on the Cherokee County Athletics Site. A complete physical packet must contain each of the following documents:

• GHSA Physical Form 
• CCSD Athletic Information Consent Form 
• Student Activity Code of Conduct
